Explanation of Forms:
The Registration Form is required with a $500 deposit to register a camper. This form includes a parent/guardian signature.
The Health Form Packet includes the necessary written documentation that must be on file for each individual at camp. There are two components: a three page health history and a one page physical exam and review to be completed by the individual’s primary care physician.
The Self Administered Packet is only for those who wish to authorize a camper to self administer the use of an epi-pen or inhaler.
The Transportation Form Packet includes information about chaperoned transportation assistance provided by camp from Portland Jetport, Boston Logan Airport, Boston Metro West or Fairfield County, Connecticut. All campers (including those arriving by auto) must have a completed transportation form on file at the camp office.
The Permissions Form provides helpful information to camp administration including best contact information and emergency contact information for parents.
The Horseback Riding Permission Form provides permission for your camper to ride and/or permission to jump while riding if the instructor feels she is ready. State of Maine Law requires this form to be complete and signed by a parent or guardian in order for your camper to participate in our riding program.
The Visitation Policy and Motels Packet is information parents and relatives need if visiting a camper. Please make sure you share it with your family. This also includes some important dates at camp and links to area chamber of commerces with lodging information.
The Important Information is just that. It contains all kinds of helpful information to prepare your camper for their summer adventures with us. It includes information about dates, travel, footlockers, mail and email.
The Packing Inventory List is a guide for packing essentials for camp. We suggest that you print one and tape it to the inside of your camper’s footlocker.
